New, incoming Pre-K students will need an up to date physical and immunization form (includes the medical history portion completed and signed by parent/guardian), dated on or after August 18, 2020. Returning pre-k students physicals are good for two years.
Kindergarten students will need an up to date physical and immunization form (includes the medical history portion completed and signed by parent/guardian), dated on or after August 18, 2020. These students will also need to submit an eye exam performed by an Optometrist or Opthamologist (dated on or after August 18, 2020) and dental exam (dated on or after November 15, 2020).
Second grade students will need a dental exam (dated on or after November 15, 2020).
Sixth grade students will need an up to date physical and immunization form (including the medical history portion completed and signed by parent/guardian), dated on or after August 18, 2020. These students will also need to submit a dental exam (dated on or after November 15, 2020).
New students to Illinois will need up to date state of Illinois physical and immunization form (including the medical history portion completed and signed by parent/guardian), dated on or after August 18, 2020. These students will also need to submit an eye exam performed by an Optometrist or Opthamologist (dated on or after August 18, 2020). New students entering kindergarten, second and sixth grade will also need a dental exam (dated on or after November 15, 2020).
Any student trying out for sports will need to submit an up to date physical that has been completed within the last 13 months. These students will also need to submit one concussion form per school year.
