Pre-K Aide Position Available for the 2022/2023 School Year


A friendly reminder from Nurse Joy and Nurse Jen:
-Any student entering Kindergarten will need an up to date ( performed on or after 7/17/21 ) health physical, dental exam, and vision exam. The health physical must be on the Illinois Certificate Child Health Examination Form (must be on the 11-15 form). Parents must complete and sign the Health History portion on the back of the form. The dental exam must be documented on the Illinois Department of Public Health Proof of School Dental Examination Form.
-All incoming second graders need an up to date dental exam by the first day of school. The dental exam must be documented on the Illinois Department of Public Health Proof of School Dental Examination Form.
-All incoming sixth graders need an up to date health physical on the Illinois Certificate Child Health Examination Form (must be on the 11-15 form). Parents must complete and sign the Health History portion on the back of the form.
Sixth graders also need a Tdap vaccine and a meningococcal vaccine if they have turned age 11.
Lastly, sixth graders need an up to date dental exam documented on the Illinois Department of Public Health Proof of School Dental Examination Form.
-ANY student who wishes to try out for a fall sport MUST have an up to date health physical OR sports physical on file BEFORE the first day of tryouts.
